Applying for Accommodations
EICC Disability Services helps students with disabilities find and implement reasonable academic accommodations. To do so, a Disability Service Provider (DSP) balances the needs of the student and the academic objectives of the program. To get this process underway, complete an application and provide documentation of the disability.
Applying for accommodations is a two-part process:
- Complete the Student Disability Services Application.
- Submit Documentation from Health Care Provider to your campus’s Disability Service Provider.
Documentation of the disability should include:
- The diagnosis of the condition
- The severity and longevity of the condition
- A description of the functional difficulties and limitations in an educational setting
- Recommendations for academic accommodations
- Signature and contact information of diagnostician including mailing address, telephone number, and email address.
Typically, high school 504 Plans or Individualized Education Plan (IEP) are not sufficient documentation for the provision of accommodations, but may be used to supplement disability information.
For learning disabilities, ADD or ADHD, traumatic brain injuries and most psychiatric disabilities, a full psycho-educational test battery should be included in the document.
Disability Services will not accept the following:
- Information that is illegible
- Information from non-appropriately licensed professionals
- Research articles
- Documentation that is provided by a member of the student’s family
Documentation from Health Provider
In order to receive accommodations, current and comprehensive verification of the disability must be provided by a qualified professional. Qualified professionals may include, but are not limited to, physicians, psychologists, audiologists or other licensed authorities as determined by the type of disability.
The documentation provided must include the diagnosis of the disability, the impact of the disability in an educational setting, the severity and longevity of the condition, and suggestions for appropriate learning strategies and accommodations.
If you, the student, have current documentation ready to submit to your campus Disability Services Provider.
If you need to acquire documentation from your provider, please download or print the following form and send it to your provider: Verification of Disability Form. This form must be returned to the DSP on your campus.
Once the disability service provider (DSP) has received both the application and the documentation of the disability, he or she will review the documentation and determine eligibility for services.
The DSP on your campus will send you an email message inviting you to schedule an initial appointment to discuss accommodations and services.
Please note that the DSP will use only your EICC email for communications—not your personal email.

After you have applied for accommodations and submitted documentation, you will be invited to set up an initial meeting with the Disability Service Provider (DSP) on your campus.
During your initial meeting, we will orient you to our process and your accommodations. Come ready to discuss your disability and how it impacts you in the educational setting.
During the initial meeting, we will:
- Give a short orientation to Student Disability Services
- Talk about our interactive process
- Go over your rights and responsibilities
- Discuss your specific accommodations in detail
- Explain how to share your Letter of Accommodations (LOA) with your instructors
- Answer questions you might have
Without the letter of accommodation, instructors are unable to provide accommodations; therefore, make sure to email it to your instructors as soon as possible. Furthermore, it is your responsibility to communicate with your instructors about your accommodations so that both you and the instructor clearly understand how the accommodations will be implemented – keep in mind that accommodations are not retroactive.
Since a new form is required each semester, email the DSP on your campus to obtain a new form at the beginning of each semester. Then email the form to your instructors before classes begin the next semester.
